Ballast/starter/lamp Damage?

i got a Fluorescent light (shorter 20W type) not working.

at 1st i thought is starter problem,then i replace the starter.it is work after changing starter,but after 3-4times on off, it damage.

then i change a new ballast and the starter this time round.again,it failed after 3-4 times on off!

what is the problem?

anybody here experience this?
