Need Help On Timber Flooring

Hi there, From what i understand, it is not normal to have protusion caused by nails as timber flooring is laid using adhesive. nails are used only if a layer of plywood is required to provide a smooth base but even then, the nails are supposed to be flushed with the plywood. Anybody correct me if i am wrong, this seems to be more of a defect. -
